Lost your forklift certificate - what now?

If you have lost your forklift certificate (proof of forklift operator training), request the replacement document directly from the training provider that delivered the original training and issued the document. No new exam is required.

Step 1 - Identify the training provider If you completed the course with a training institute, an inspection body or a manufacturer's academy, contact that organisation directly. The contact details are often still known from the original registration or can be found on the provider's website.

Step 2 - Gather supporting documents The following documents are usually required:

• Copy of your ID (identity card or passport)
• Details of the training date and location (can be reconstructed from old e-mails or payslips)
• A loss report if applicable (formally required by some providers)

Step 3 - Request the replacement document and pay the fee Most providers issue the replacement document for an administration fee. Cost: usually 10-50 euros. Processing time: 5-20 working days, depending on the provider and how the records are archived.

If the original training provider has gone bankrupt or ceased trading:

Request the archive from the legal successor Many training providers are taken over - the legal successor often keeps the records. Search the trade register or ask industry associations.

Consult the Netherlands Labour Authority For questions about the applicable requirements for the competence of forklift operators, the Netherlands Labour Authority (Nederlandse Arbeidsinspectie) can provide information in some cases.

Complete new training If no supporting documents can be obtained, the last resort is to repeat the training. Experienced operators can consider a shortened course.

No - at least not without demonstrable competence. Under Dutch occupational health and safety legislation (Working Conditions Act and Working Conditions Decree), the employer must ensure the demonstrable competence of the forklift operator. Without a document, the employer will find it difficult to demonstrate that competence.

Until the replacement document is issued, the employer can record internally that the qualification demonstrably exists (e.g. through statements from supervisors, old payroll records, course confirmations in the personnel file). This is not a legal basis but a pragmatic interim practice that should be agreed with the prevention officer.

Losing the certificate can largely be avoided with simple measures:

Choose the card format: many providers issue the certificate in a laminated card format (on request or for a surcharge of 5-20 euros). The format is handier and more durable.

Make a digital copy: scan or photograph the document immediately after receipt and store it securely.

Central company archive: employers with several forklift operators should keep copies of all certificates centrally in their health and safety documentation.
